Routinely monitor levels within 72h of starting therapy with systemic flucytosine.

Pre-dose (trough) clotted blood (serum) samples should be taken just before a dose of flucytosine.

Pose-dose (peak) clotted blood (serum) samples should be taken 30 minutes after an intravenous dose, or  2 hours after an oral dose.

The dose and interval should be adjusted in order to produce target peak and trough serum concentrations.

Expected levels, according to 2017 Bristol Antimicrobial Reference Laboratory – Guideline Ranges:

    Pre-dose (trough) target level: 20-40 mg/L

    Post-dose (peak) target level: 50-100mg/L

Pre-dose (trough) concentrations <20 mg/L have been associated with treatment failure and emergence of resistance.

Post-dose (peak) concentrations >100 mg/L have been associated with toxicity.
